Authorization for Cremation & Disposition

Please click the link below to download and view the Authorization for Cremation & Disposition document.  This is one of the documents that New York State requires to be signed and initialed, and presented to the crematory by your funeral director as a formal request for cremation.  Your funeral director will provide and fill out the document at the arrangement conference.  This document may not be completed before you meet with him or her.
